1. Upload the records
Drop in the case medical records as a PDF, one file or a batch from multiple providers. Digital (text-based) and scanned PDFs both work.
2. Extract every event, cited to its page
Chartely reads the document text page by page and extracts every clinically significant event, visits, imaging, procedures, medications, work-status notes, with a date, provider, facility, a plain-English summary and a citation to the exact page it came from.
3. Normalize, order and flag gaps
Dates are normalized where the source allows it, every event is sorted chronologically across providers, and pure date-math flags any gap of 45+ days between consecutive dated events, no model guesswork, just arithmetic you can verify against the citations.
4. Review, export or integrate
Review the timeline and narrative draft, then export to Word for filing, CSV or JSON for your own analysis, or call the same engine from your own code or an AI agent via the API and MCP server.

Questions about the process
Does it work with scanned or handwritten records?
Yes. Chartely handles both digital PDFs and scanned records. Poor-quality scans and handwriting generally produce a lower confidence score, always review flagged entries before relying on the output.
What if a page can't be read as a medical record?
You'll see a typed error explaining why, rather than a silent, guessed-at result. A page that isn't clearly a medical record is flagged, not fabricated.
Can I process records from more than one provider at once?
Yes, upload the full case record set in one batch and events from every provider are extracted and interleaved on a single chronological timeline.
How long does building a chronology take?
Most record sets process in minutes, not the 5-10 business days an outsourced chronology service typically takes. Larger files or lower-quality scans can take a little longer.
